5/30/2023 EnchantaVaughn Approved 1.Behavior: Upon QMHP-Câs arrival, Ben-Nita was focused during session as evidenced by her openly responding to QMHP-C questions. Ben-Nita was well dressed and presented during the session. Ben-Nita was able to cooperate and communicate her feelings well during the session. 2. Behavior: Ben-Nita displayed good interaction and contributing her thoughts to the conversation. Ben-Nita completed the psycho educational activity without being distracted and QMHP-C praised her for being able to concentrate and focus while being outside. 1.Intervention: QMHP-C observed Ben-Nita was alert and paying attention during session. QMHP-C observed Ben-Nita following directions and completing at home tasks with no prompting or complaining. QMHP-C observed Ben-Nita displaying positive social interaction with sibling by sharing and playing safely. QMHP-C processed with Ben-Nita about her day at school. QMHP-C praised Ben-Nita for her positive behaviors and verbalized to Ben-Nita that she is doing an amazing job with following through with orders and maintaining a good attitude. QMHP-C encouraged Ben-Nita to keep up the good work and continue to listen to her mom, as well as, trust herself with every decision that she makes on her own. 2. Intervention: QMHP-C assisted Ben-Nita to behave in more positive ways by addressing the conflict she was facing with her mom. QMHP-C informed Ben-Nita of replacement behaviors in efforts to decrease attention seeking behaviors Ben-Nita often is involved in. QMHP-C modeled appropriate ways for Ben-Nita to get positive attention from her family. QMHP-C inquired of Ben-Nitasâ current mood. QMHP-C utilized active listening skills as Ben-Nita spoke of her current mood. QMHP-C assisted Ben-Nita in exploring other coping strategies that she had previously used to solve other problems. QMHP-C addressed all negative and uncooperative behaviors such as not following directions or doing what she is told. QMHP-C explained to Ben-Nita that failing to comply to rules is a form of disrespect. QMHP-C reminded Ben-Nita that there are consequences to her actions that could result in punishment at home or being banned from a community setting. QMHP-C discussed various strategies with Ben-Nita to assist her with making progress towards her goals and objectives. 1. Response: Ben-Nita transitions smoothly and safely to session area, displaying appropriate behaviors. Ben-Nita communicated thoughts and feelings well during session. Ben-Nita made constant direct eye contact with authority figures during outing. Ben-Nita was pleased to be praised for her behaviors. Ben-Nita stated that she felt she was smart and kind to her family. Ben-Nita communicated that it is often hard to remain positive at all times about herself when she doesnât always have the best things or necessities that she needs. Ben-Nita discussed numerous situations where she had to adapt even when she didnât want to. Ben-Nita expressed that she tries to think highly of herself. 2. Response: Ben-Nita communicated that she feels that she will be better behaved, due to having a woman as a counselor. Ben-Nita followed all directions during the session. Ben-Nita acknowledges expectations and reviews goals and objectives with counselor and parent daily. Ben-Nita was acceptive of prompts given by mom and counselor. Ben-Nita agreed to listen to the adult and follow all rules no matter if she sees someone else breaking them. Ben-Nita is reminded to tell an adult if someone is interrupting her learning in anyway. Ben-Nita was doing a good job ignoring distractions and maintaining appropriate behavior in the session. Ben-Nita did not display any outbursts during the role play exercise. Ben-Nita is making progress with expressing herself. Ben-Nita made insightful comments which contributed to discussion. Ben-Nita responds well to praise and support. Ben-Nita is acceptive of redirection., sibling, and friend. Ben-Nita incorporated numerous strategies that she learned from her previous sessions that she felt worked best for her with meeting her objectives.
